Support Services for Pregnant Women & Their Children

Family Connects Oregon is a free evidence-based, nurse home visiting program for all families with newborns. 

The Family Connects Oregon program supports parents by linking nurses, community resources, and families. Our mission is to connect you with resources that nurture your whole family and support your child. It’s about planting seeds for the future. Together, we’re growing healthy babies!

Family Connect Nurses partner with families to provide:

  • 1-3 post-partum visits at around 3 weeks of age
  • Maternal and Newborn Assessment
  • Lactation support
  • Postpartum and Newborn education
  • Linkages to Maternal Mental Health support and other resources in the community

Babies First!  is a nurse family support service program for pregnant women and children up to age 5. Nurses visit you and your baby in your home or at another agreed upon location. Nurses can:

  • Support you during your pregnancy 
  • Support you with breastfeeding or feeding concerns
  • Weigh your baby, as well as help monitor growth and development
  • Connect you to needed services you may need
  • Help with making your home safe for your child
  • Help you keep up with well child care, immunizations, dental, vision, etc.

Support Services for Families with Special Needs Children up to age 21

CaCoon (CareCOOrdinatioN) support services are available for families with children with special health needs from birth to age 21. Children who have chronic health or developmental conditions are eligible. Let us help you navigate the health care system in hopes of reducing stress; we can also help you gather information to aid in making the most informed decisions for your child.  Let us help facilitate your health care team in providing patient centered care.
